APPOINTMENT, PAYMENT & PRIVACY POLICIES

Making An Appointment is easy. 

Simply call the office and I'll work with you to arrange a convenient time. You can also book online by clicking the button below.

Appointments are considered a contract for services offered at a quoted price. Failure to keep an appointment is a breach of this simple contract. Policies below apply to all appointments.

NEW CLIENTS

All new clients are required to provide credit - debit card information to guarantee an initial appointment.  No charges are made unless you are a NO SHOW or LATE CANCEL.

Once you are a established (returning) client, you can make appointments online.

LATE CANCELLATION-NO SHOW POLICY

Once you have scheduled an appointment, if you need to cancel or reschedule, 24 hours prior notice ia required to avoid a charge. 

LATE CANCELLATION or NO SHOW FEE

is 50% of the STANDARD FEE for THE service and time booked. SEE FEE SCHEDULE PAGE under the Services Menu above.

• CASH OR CHECK PREFERRED
• GIFT AND PRE-PAID vouchers
• CREDIT - DEBIT CARDS 
• FSA, HSA, HRA DEBIT CARDS

PAYMENT POLICY

• NEW CLIENTS are required to prepay with Credit-Debit card
• Payment is due at time of service
• IF YOU INCUR A LATE CANCELLATIO OR NO SHOW FEE, YOU WILL BE BILLED

OTHER PAYMENT OPTIONS:  INSURANCE

In New York State, Massage Therapy is not generally covered by Health Insurance. It is covered under>:

• NO FAULT AUTO INSURANCE 
• WORKERS' COMP INSURANCE

if prescribed by a doctor as part of treatment. I can work with you and your PCP to secure coverage, but coverage is NOT GUARANTEED. 

PLEASE NOTE THE FOLLOWING:

• Depending on your Insurance carrier, Insurance assignment is possible. No out-of-pocket cost.

• Otherwise, you will be charged a reduced fee and a detailed statement for insurance re-imbursement will be provided.

PRIVACY POLICY

As a healthcare provider, Nick Clidas Massage and Wellness is bound by the same privacy rules as your doctor, counselor or attorney, under the Federal Health Information Privay and Portability Act (HIPPA) regulations.

Your email address and phone number are collected for communication pruposes only including appointment confirmation, reminders and   newsletters, announcements and special offers.

All personal information, health and treatment information is encrypted and stored as required, in digital format behind multiple firewalls.  

Only you and your Massage Therapist can access this information.
